VERMONT FAQs

  • A medical patient must be 18 years of age or older
  • Only patients who can prove West Virginia residency can apply for the medical program (personal driver’s license, Government ID)
  • West Virginia residents must also have medical records showing a diagnosis that qualifies for medical cards by a licensed doctor.

  • Severe chronic or intractable pain
  • Spinal cord damage
  • Sickle cell anemia
  • Any Terminal illness

  • The West Virginia state program will charge $50 patient ID card application fee, which must be paid by check or money order
