A medical patient must be 18 years of age or older

Only patients who can prove West Virginia residency can apply for the medical program (personal driver’s license, Government ID)

West Virginia residents must also have medical records showing a diagnosis that qualifies for medical cards by a licensed doctor.

Medical Patients with the following conditions to apply for a West Virginia medical card:

  • HIV/AIDS
  • Epilepsy
  • Huntington’s disease
  • Intractable seizures
  • Multiple sclerosis
  • Neuropathies (chronic nerve pain)
  • Parkinson’s disease
  • Post-traumatic stress disorder
  • Severe chronic or intractable pain
  • Spinal cord damage
  • Sickle cell anemia
  • Any Terminal illness

The West Virginia state program will charge $50 patient ID card application fee, which must be paid by check or money order
