  • You must be able to provide proof of your Illinois residency
  • You cannot have a Commercial Driver’s License (CDL)
  • You must have a diagnosis that qualifies in Illinois
  • You must submit a signed Physician Certification with your IDPH application – and that is where we come in! We will provide the Physician Certification and then you can apply to get your card.
