To be eligible for a medical card you must:

  • Be a Colorado resident
  • Be 18 or older
  • Have a qualifying medical condition

  • Debilitating medical conditions 
    • Cancer
    • Glaucoma
    • HIV or AIDS
    • Cachexia
    • Persistent muscle spasms
    • Seizures
    • Severe nausea
    • Severe pain
  • Disabling medical conditions
    • Post Traumatic Stress Disorder (PTSD)
    • An Autism Spectrum Disorder
    • Any condition for which a physician could prescribe an opioid

  • The processing fee for application submission amounts to $29.50, and it remains non-refundable. This fee is mandatory with every application submission, playing a crucial role in covering the expenses associated with maintaining the Registry. The funds collected from this fee contribute to sustaining various services, including customer and technical support.
